Our new survey is:
Individual
You don't need to divide the hours between members of a group, simply report on all visits and hours for which you were present.
Quarterly
Please report on all activity within the three month period for which you are submitting. Please submit activity by noon on the third Friday of the month following each quarter (e.g. April 19 for the first quarter).
Caffeinated
All FCNs and HMs who complete the form below will be entered into a quarterly drawing for a $25 gift card to Woods Coffee. There is no minimum hours requirement. Thank you to Woods Coffee for donating the gift cards!
Online
You will receive quarterly email reminders to submit your hours. Check it out on our website anytime at www.healthministriesnetwork.net/hours.
